Adoption Contract — AAVA Rescue

This agreement is entered into by and between Animal Aid for Vermilion Area ("AAVA") and the applicant ("Adopter") for the adoption of the identified rescue animal ("the animal").

By completing and submitting this form, the Adopter agrees to the following terms and conditions. Fields marked with * are required.

Agreements

1. Lifetime commitment. The Adopter agrees to provide the animal with adequate food, water, shelter, and veterinary care for the duration of the animal's life.

2. Indoor/supervised living. The Adopter agrees that the animal will live indoors or in a safe, secure environment and will not be left chained or tethered unattended.

3. No transfer without consent. The Adopter agrees not to sell, give away, or otherwise transfer ownership of the animal without first notifying AAVA and offering to return the animal to AAVA.

4. Veterinary care. The Adopter agrees to maintain the animal's vaccinations and seek veterinary care when needed.

5. Return policy. If the Adopter can no longer care for the animal, the animal must be returned to AAVA before being placed elsewhere. Adoption fees are non-refundable.

6. AAVA right to reclaim. AAVA reserves the right to reclaim the animal if, in AAVA's judgment, the animal is being mistreated or the terms of this agreement are being violated.
